Position Summary:

• The Behavioral Health Case Manager provides services designed to support and maximize the ability of patients to better direct their health care experience through the use of supportive interventions, psycho-educational interventions, linkage to health plan and community resources and other activities geared at maintaining or enhancing the patient's coping capacity and stability. The Behavioral Health Case Manager also provides linkage and coordination with other care providers in the community or within TrueCare.

  Required Qualifications:

  • High school diploma or equivalent.
  • Minimum (1) one year working in a clinical setting.
  • Minimum (1) one year working with behavioral health population.
  • Bilingual in English and Spanish.
  • Computer proficiency with Microsoft Office suite products (Outlook, Word, Excel, and PowerPoint) and electronic health record systems.
  • Knowledge of local and national resources related to health care.
  • Preferred Qualifications:
  • Associate's degree in social services, mental health, psychology, human services or related field.
  • Experience with brief interventions and case management techniques.
  • Experience working with low income, diverse population and persons impacted upon by mental illness.
  • CCMC (Certified Case Manager Certification).

    Licenses:

    May be required to travel between all TrueCare sites and locations therefore CA Driver's License and clean driving record required!

If you are hired at TrueCare, your final base salary (within the pay range), will be determined based on factors such as education, knowledge, skills, and experience. In addition to those factors, we believe in the importance of pay equity and consider the internal equity of our current team members when determining any final offer. Please keep in mind that the range mentioned above is the full base salary range for the role. Hiring at the maximum of the range would not be typical to allow for future and continued salary growth.     Employment is contingent on having an annual COVID-19 vaccination, or use of a mask, from November 1 through March 31 of every year.
