Adapt Integrated Health Care is looking for a School-Based Therapist to join their Behavioral Health team!

The School Based Therapist, a QMHP position, provides integrated outpatient mental health treatment services in a Child Day Treatment setting that are based on medical necessity and that support and promote mental health recovery and reintegration into regular educational settings. The School Based Therapist provides individual, family, and group therapy for youth who have been referred by a Douglas County school district and who meet the qualifications for psychiatric day treatment. This therapist also provides case management and consultation to Douglas County school staff for referral in and reintegration back into regular educational settings. He/she will manage the therapeutic milieu by directing/teaching therapeutic and behavioral interventions to the mental health staff in the school base setting. He/she will manage the State documentation and review requirements for each youth in the school base setting.
Adapt Integrated Health Care QMHPs endeavor to a) Promote recovery, resiliency, wellness, independence and safety for individuals receiving addictions and mental health services and supports; b) Specify standards for services and supports that are person-directed, client-guided, culturally competent, and trauma-informed and wellness informed; and c) Promote functional and rehabilitative outcomes for individuals throughout a continuum of care that is developmentally appropriate.
QMHPs responsibilities are to engage and respond in crisis intervention; facilitate access for mental health treatment at Adapt; provide therapeutic interventions including informed consent, advocacy, and integrated care; assist with access to community resources; facilitate development and retention of natural and community supports; provide appropriate individual, group, and community psycho-education; actively work to reduce stigma, barriers, and to build a community foundation for success.
QMHPs accomplish these responsibilities through assessment and prescription of treatment services that support rehabilitation, safety, and recovery through measurable outcomes in collaboration with clientele, and their natural supports when appropriate. Some of the interventions provided include comprehensive assessment; collaboratively developed Individual Service and Support Plan; individual and group therapy and skills training and development; case management; referrals, consultation, coordination and collaboration with community partners and providers; Discharge Summary and continuity of care through transfer and referral. QMHPs as well as other treatment providers at Adapt endeavor to support and promote positive outcomes through use of Evidenced-Based, Best Practices whenever possible.
QMHPs adhere to the OARs; agency standards and practices; and follow their board in regard to ethical standards of practice.
Qualifications
Required Qualifications:
- Master’s Degree in Social Work, Psychology, behavioral science, or related field,
- Experience in a mental health supported educational setting is preferred.
- Must possess valid Oregon driver's license
