Summary:
The Case Management Assistant supports case managers by performing administrative duties, coordinating care plans, and facilitating communication between patients, families, and healthcare providers.
Responsibilities:
- Coordinates follow-up appointments for patients.
- Assists with the discharge planning process, including arranging home care services, skilled nursing facilities, durable medical equipment, etc.
- Provides administrative support such as scheduling meetings and managing correspondence.
- Participates in ongoing training and development activities.
- Delivers regulatory forms to patients/authorized representatives.
- Scans and enters documentation into EMR and updates document-controlled forms.
- Orders supplies and manages finances for the department.
- Answers phone calls and redirects to the appropriate extension.
- Manages department mail, faxes, and emails.
- Prepares minutes from meetings.
- Assists with Joint Center Orientation.
Other information:
Required:
- High school diploma or equivalent.
- One (1) year of administrative experience in a healthcare setting.
- Proficiency in using electronic medical recordkeeping (EMR) systems.
- Basic Life Support (BLS) for healthcare provider certification.
- Strong organizational and communication skills.
