A Behavioral Health Support Specialist (BH Support Specialist) works in the primary care clinic as an integral team member and functions as a member of the Behavioral Health Integrated Services team to provide integrated support to patients with multiple medical and/or co-occurring mental health and/or substance abuse disorders. Advocates for and supports the patient, engages with community agencies/health care providers and others on his or her behalf to ensure access to services needed to increase wellness self-management and reduce emergency room visits and/or hospitalizations. Provides clinical support to the team by providing updates on patient access to care and resources to maintain focus on outcomes and best practices.
.ESSENTIAL DUTIES AND RESPONSIBILITIES
- Assists Behavioral Health Consultants with complex patient cases to ensure access to care.
- Assists Behavioral Health Consultants with crisis situations, i.e., checking on bed availability, providing current resources to ensure patient safety.
- Ensures case management codes are properly input into the patients’ electronic health record.
- Follows up with patients and referring agencies to determine access/barriers to care.
- Assists Behavioral Health Consultants with updating resource materials and information.
- Remains up to date on resources within the community.
- Builds partnerships with referral sources and community agencies.
- Performs other duties as assigned.
QUALIFICATIONS
Minimum Qualifications:
- Bachelor’s Degree: Social Work, Psychology, or another related field; or related experience.
- Computer Literacy
Preferred Qualifications:
- Bilingual Preferred: Spanish/English
